ITSC would like to announce the planned retirement of MyCUForm. As part of this transition: 

Phase 1 – New user and Form creation will no longer be supported by the end of August 2026

Phase 2 – The service will be fully retired by the end of December 2026 

Please ensure that all data is backed up before the service is closed to avoid any data loss.

Recommended Alternatives 

To support a smooth transition, users are encouraged to consider the following form creation platforms:

• Microsoft Forms – Recommended as the first choice for general form creation due to its ease of use and integration with Microsoft 365.
• Qualtrics– Offers advanced form creation tools for more complex survey and data collection needs.
• CUWebForm – Suitable for departmental use; access requires approval from the designated coordinator.

Users are reminded not to create new forms and download data before the system termination date (31 December 2026).
